| What are SMC Therapy Programs? | ||

SMC programs are “needs approach” functional rehabilitation. Programs are specifically designed to meet the individual physical, education and emotional needs of patients and families. What is Functional Rehabilitation? Functional rehabilitation addresses the specific problems that lead to or cause an inability to perform certain activities, or when performed may be unsafe or create risk for injury. Our programs provide one-on-one therapy that addresses the basic necessities of life. Walking, balance, fall prevention, changing positions, fine motor dexterity, self-care, independent activities, speech and safe swallowing may be some of the areas of focus in the patient’s treatment plan. Senior Medical Care programs are available for a variety of diagnoses such as arthritis, low vision, orthopedic problems, neurological disorders including stroke, Parkinson’s disease and multiple sclerosis to name a few. Functional Rehabilitation programs begin with a skilled therapy assessment followed by treatment that combines neuromuscular re-education and task oriented compensatory techniques. The goal of functional rehabilitation is to improve independence and quality of life. Specific interdisciplinary programs are available such as: What services are provided? Senior Medical Care programs provide: Education and Safety Programs Home Therapy Services Resource Information-Support Groups-Community Services Educational and Safety Programs Education programs are integrated with therapy services and provide specific information about disease and disorders. Therapists instruct patients in the use of adaptive equipment and compensatory techniques. Safety programs address falling, choking or swallowing difficulty and safety awareness in the home environment. Social service information assists patients and families in coping with changes they face as a result of loss of mobility. |
